Audit of the code to automate the creation of the integration binning scheme. Fix one bug, some tidy-ups, and reintroduce a couple of lost features: use of a single square-DP grid if a narrow resonance is found in m12 and extension of regions down to threshold if the narrow resonances is close to threshold.
